Below is the full text of the Participation Agreement, Accident Waiver, and Release of Liability you'll sign during registration. Nothing on this page is special, it's the exact language that appears on the registration form.
PARTICIPATION AGREEMENT, ACCIDENT WAIVER & RELEASE OF LIABILITY
ASSUMPTION OF RISK
I understand that football, including full-contact drills in the Youth Football Camp, inherently carries risk of injury including but not limited to sprains, fractures, concussions, and other serious injuries. I have weighed these risks and voluntarily agree to my child's participation in Coach Ram's Future Stars Camps & Clinics (the Camp).
MEDICAL AUTHORIZATION
In the event of an emergency when a parent or legal guardian cannot be reached, I authorize the Future Stars staff, Pittsburg Unified School District personnel, and any attending medical professional to administer emergency care, treatment, and hospitalization as necessary. I understand that reasonable efforts will be made to contact me before any non-emergency treatment is given.
RELEASE OF LIABILITY
I release, waive, and discharge Coach Ram's Future Stars Camps & Clinics, Charlie Ramirez, the Pittsburg Unified School District, and their coaches, staff, volunteers, and venue partners from any and all liability, claims, or demands resulting from any injury, loss, or damage sustained by the participant during camp, except where caused by the gross negligence or willful misconduct of the released parties.
MEDICAL DISCLOSURE
I have disclosed all relevant medical conditions, allergies, and current medications on this form. I will promptly notify camp staff of any changes before or during camp.
CODE OF CONDUCT
I agree that my child and I will follow all camp rules, respect the coaching staff and fellow campers, and accept that repeated misconduct may result in removal from camp without refund.
ELECTRONIC SIGNATURE
By typing my full legal name and submitting this form, I affirm that I am the parent or legal guardian of the participant, that the information provided is accurate and complete, and that I have read and agree to the terms of this Participation Agreement.
PHOTO & VIDEO RELEASE (OPTIONAL)
I grant Coach Ram's Future Stars Camps & Clinics the right to use photographs and video of my child taken during camp for promotional purposes on the website and social channels. This consent is optional and can be withdrawn at any time by written request to info@coachramcamps.com.
